【Contraindications】Pinellia is pungent, warm, and drying. People with excessive internal heat, such as those with “dry cough, heat phlegm, or dry phlegm,” should use it with caution, or combine it with heat-clearing herbs (such as Coptis or Scutellaria).

Pinellia is an excellent Chinese medicinal herb for drying dampness, transforming phlegm, relieving cough, and alleviating wheezing. It is the dried tuber of a plant in the Araceae family. There are four main types: raw Pinellia, Qing Ban Xia, ginger-processed Pinellia, and Fa Ban Xia.

Pinellia is pungent and warm in nature and flavor, and is toxic. It enters the Spleen, Stomach, and Lung meridians. Raw and fresh Pinellia are mildly toxic, so Chinese medicine manufacturers and practitioners use products that have been processed. After processing, it can be decocted or used in soups.

Pinellia is pungent, dispersing, warm, and drying. It is toxic and relatively potent, and is especially effective at dispelling phlegm and dampness from the Spleen and Stomach. It dries dampness and transforms phlegm, directs rebellious qi downward and stops vomiting, dissipates focal distention and nodules, inhibits gastric acid secretion, promotes bile secretion, has antitumor, antiarrhythmic, sedative and hypnotic, lipid-lowering, antibacterial, anti-inflammatory, immune-enhancing, and diuretic effects.

It is an essential herb for treating “phlegm-dampness, cold phlegm, and vomiting.” It may be used for disorders caused by phlegm-dampness, especially when accompanied by cold; when heat is also present, it should be combined with bitter-cold herbs. It is used for damp phlegm and cold phlegm, cough and wheezing with abundant phlegm, dizziness and palpitations caused by phlegm retention, dizziness caused by wind-phlegm, headaches caused by phlegm syncope, vomiting and acid regurgitation, chest and epigastric focal distention, globus sensation, and more. Applied externally, it can eliminate toxins, dissipate nodules, and reduce swelling, and may be used for goiters, phlegm nodules, abscesses, and swelling.

Differences

Raw Pinellia: Best suited for external use and has a stronger ability to eliminate toxins, dissipate nodules, and reduce swelling.

Qing Ban Xia: Made by processing raw Pinellia with alum; it has a stronger ability to dry dampness and transform phlegm.

Ginger-processed Pinellia: Made by processing raw Pinellia with alum and ginger juice; it is more effective at warming the middle, transforming phlegm, directing rebellious qi downward, and stopping vomiting.

Fa Ban Xia: Made by processing raw Pinellia with limewater and licorice; it is more effective at warming and transforming cold phlegm and harmonizing the Spleen and Stomach.

For cough with a deep, heavy sound caused by phlegm-dampness obstructing the Lungs, with thin, white phlegm, it is often used with tangerine peel and Poria to enhance its ability to dry dampness and transform phlegm.

For cough and wheezing caused by cold fluid retention, with abundant, clear, thin, frothy phlegm, aversion to cold, and a cold back, it is often used with Asarum and dried ginger, which warm the Lungs and transform fluid retention.

For dizziness and palpitations caused by phlegm retention, dizziness caused by wind-phlegm, or, in severe cases, vomiting of phlegm and fluids, and headaches caused by phlegm syncope, it may be combined with Gastrodia and Atractylodes to transform phlegm, extinguish wind, strengthen the Spleen, and dispel dampness.
For vomiting caused by Stomach qi deficiency: combine with Ginseng
For vomiting caused by Stomach heat: combine with Coptis
For vomiting caused by Stomach yin deficiency: combine with Ophiopogon and Dendrobium

Tips

Er Chen Decoction

First recorded in 《Formulary of the Taiping Welfare Pharmacy》, it has the effects of drying dampness and transforming phlegm, regulating qi, and harmonizing the middle. Taiwanese Chinese medicine practitioner Ni also praised this formula as a classic.

Indications: Phlegm-dampness pattern. Cough with abundant phlegm, white phlegm that is easy to expectorate, nausea and vomiting, chest and epigastric focal distention, heavy and fatigued limbs, or dizziness and palpitations, with a white, slippery, or greasy tongue coating and a slippery pulse.

Formula: “Pinellia 15g, tangerine peel 15g (modern formulations use aged tangerine peel), Poria 9g, honey-fried licorice 5g; rinse, then add about 4 bowls of water, 7 slices of ginger, and 1 smoked plum.” Soak for about 30 minutes, decoct until reduced by half, strain, and take one dose daily in 2 warm divided doses.
Contraindications: Not suitable when there are no signs of phlegm retention; do not use for phlegm-heat congesting the Lungs, or for blood-streaked phlegm due to yin deficiency with dryness and heat.

 

Pinellia, Gastrodia, and Atractylodes Soup (relieves cough, transforms phlegm, and dispels wind-dampness)

Lean meat 100g, processed Pinellia 9g, Gastrodia, Poria, and tangerine peel 6g each, Atractylodes 15g, licorice 3g, 2 slices of ginger, and 2 pitted red dates. Add water and simmer for 60 minutes; season to taste.

Pinellia, Magnolia Bark, and Poria Soup ( transforms phlegm, dissipates nodules, directs rebellious qi downward, and harmonizes the Stomach)

Lean meat 100g, processed Pinellia 12g, Magnolia bark 9g, Poria 12g, fresh ginger 9g, and perilla leaf 9g. Add seven liters of water, boil until reduced by half, divide into four warm doses, taking three during the day and one at night.

Pinellia, Ginseng, and Licorice Decoction ( dissipates nodules, relieves focal distention, directs rebellious qi downward, and stops vomiting)

Lean meat 100g, processed Pinellia 12g, Scutellaria, dried ginger, and Ginseng 9g each, Coptis 3g, 10 pitted red dates, and licorice 9g. Add 7 liters of water, boil until reduced by half, divide into four warm doses, taking three during the day and one at night.
