Bupleurum is also known as minor bupleurum, di xun, shan cai, gu cao, and chai cao. It is the dried root of Bupleurum chinense or narrow-leaved bupleurum, plants in the carrot family.It is an herb that releases the exterior and treats wind-heat.
It has a bitter taste and is slightly cold in nature, entering the liver, gallbladder, and lung channels.
Bupleurum can both disperse pathogenic qi in the gallbladder channel to harmonize and reduce fever, and disperse stagnant qi in the liver and gallbladder channels to soothe the liver and relieve depression. It also raises clear yang qi in the liver and gallbladder to treat sinking disorders, making it an essential herb for the liver, gallbladder, and lung channels. The Divine Farmer's Classic of Materia Medica further lists it among the superior-grade Chinese medicinal herbs.。

This formula primarily treats: lesser-yang syndrome in exogenous febrile disease, characterized by alternating chills and fever, a feeling of fullness and discomfort in the chest and hypochondria, taciturnity and lack of desire to eat, irritability with a tendency to vomit, a bitter taste in the mouth, dry throat, dizziness, a thin white tongue coating, and a wiry pulse; heat entering the blood chamber, characterized in women by exogenous febrile disease with menstruation just ending and periodic attacks of chills and fever; as well as jaundice, malaria, and various internal medical disorders presenting with lesser-yang syndrome.
In traditional Chinese medicine clinical practice, it is commonly used to treat influenza, malaria, chronic hepatitis, acute and chronic cholecystitis, gallstones, pleurisy, otitis media, puerperal fever, acute mastitis, orchitis, gastric ulcers, and other conditions involving pathogenic factors lodged in the lesser yang with disharmony between the gallbladder and stomach.

- Chest and hypochondriac distension and pain: A feeling of fullness or stabbing pain below the ribs on both sides and in the chest; the pain often fluctuates with emotional changes (worsening when angry and easing slightly after sighing).

Original dosage: Bupleurum 6 g, cyperus 4.5 g, stir-fried bitter orange 4.5 g, Sichuan lovage root 4.5 g, white peony root 4.5 g, tangerine peel 6 g, honey-fried licorice root 2.5 g.
Modern dosage: When prescribing in practice, modern TCM practitioners often increase the dosage of the entire formula proportionally according to an adult’s constitution (for example: bupleurum 10–12 g, prepared cyperus 10 g, stir-fried bitter orange 10 g, Sichuan lovage root 10 g, white peony root 12 g, aged tangerine peel 10 g, honey-fried licorice root 6 g).
- If qi stagnation transforms into fire (manifested as a bitter taste in the mouth, irritability, and a red tongue): Add gardenia fruit and Scutellaria root to clear heat.
- If stomach qi rebels upward (persistent belching, nausea, and vomiting): Add Inula flower and hematite to promote qi movement and direct rebellious qi downward.
- If abdominal or stomach pain is pronounced: Add corydalis rhizome and Sichuan chinaberry fruit to enhance the pain-relieving effect.
- If breast tenderness and pain are severe: Add green tangerine peel, turmeric rhizome, and cowherb seeds to promote liver qi flow and unblock the channels.
- Soak in cold water: Put all the herbs in a pot with 500–600 milliliters of water and soak for about 30 minutes first. This allows the herbs to absorb enough water, making it easier for the active ingredients to dissolve during decoction.
